Custodian of Records - Public Records Request / Body Shipment / Port of Entry Letters

The Custodian of Records coordinates the processing of requests for records made pursuant to the California Public Records Act (CPRA) received by the Department of Public Health (DPH).

Please review the Frequently Requested Records section below to see if the records you are seeking are available online. Please note that requests for information (e.g., questions about a specific Public Health matter), as opposed to records are not subject to the CPRA and should instead be directed to DPH’s Infoline at (833) 540-0473. Informational inquiries will be forwarded to the appropriate DPH staff member for a response.

FREQUENTLY REQUESTED RECORDS
Environmental Health Inspection Reports and reports of facilities closed by Environmental Health are available online and do not require a CPRA request. To access these online records, please click on the links below:

ENVIRONMENTAL HEALTH INSPECTION RECORDS – This link contains inspection reports for the previous 5 years for all facilities, including Restaurants/Food Markets, Apartment Buildings, Condominiums, Swimming Pools, Food Truck, Camps, and Cannabis Facilities.

ENVIRONMENTAL HEALTH – FACILITY CLOSURE LIST – This link provides facility health inspection records that resulted in a closure of a facility during the past 100 days.

Hazardous Materials Records: All Hazardous Material inspection report requests must be made to the Los Angeles County Fire Department, Health Hazmat Division. Please access the Fire Department's website for instructions on submitting a record request: https://fire.lacounty.gov/public-records-requests/

CALIFORNIA PUBLIC RECORD ACT REQUESTS
The public may make a CPRA Request by submitting this CPRA Records Request Form: http://publichealth.lacounty.gov/phi/docs/COR-Online-Form.pdf to DPH. The Custodian of Records will respond to requests within 10 days of receipt.
